• Candidates must submit their application in the standard form of application published in the Assam Gazette Part-IX.

  • Fill the form in Block Letters using Black/Blue ball pen only.
  • Paste one recent passport size colour photograph in the specified box.
  • Attach two additional passport size photographs with the application.
  • Put signature in the designated signature box.
