Mukhya Mantri Lok Sevak Arogya Yojana (MMLSAY) is a health reimbursement scheme launched by the Government of Assam to provide financial assistance for medical expenses to state government employees, pensioners, and their dependents. The scheme aims to streamline the medical reimbursement process through an online portal, ensuring timely and efficient processing of claims.

  • Pensioners:
    • They pay a one-time lump sum amount equivalent to 10 years of subscription according to their last held post.

Empaneled Hospitals

  • Assam government hospitals.
  • Select private hospitals across Assam empaneled under MMLSAY.
  • Treatment in non-empaneled hospitals is allowed in emergency cases, but requires documentation.

  • Aadhaar Card: For identity verification.
  • PAN Card: Used as a primary identifier for employees and pensioners.
  • Passport-sized Photograph: Recent photo for identification purposes.
  • Address Proof: Such as an electricity bill or domicile certificate.
  • Employment Details: Latest salary slip for employees or Pension Payment Order (PPO) for pensioners.
  • Mobile Number and Email ID: For communication and updates.
  • Dependent Documents: If applicable, documents proving the relationship with dependents.

Required Documents for Medical Reimbursement Claims

For submitting a medical reimbursement claim under MMLSAY, the following documents are generally required:​

  • Discharge Summary/Certificate: From the hospital where treatment was received.
  • Hospital Bills: Final bills and summaries issued by the hospital.
  • Medical Prescriptions and Reports: Including diagnostic reports and doctor’s prescriptions.
  • Essentiality Certificate: Certifying the necessity of the treatment.
  • Referral Certificate: If treatment was referred by another medical board or authority.
  • Bank Passbook Copy: For processing the reimbursement payment.
  • Self-Declaration: Stating that no claim has been made from other sources.
  • Dependent Certificate: If the claim is for a dependent, proof of dependency is required.
  • Leave Order: If applicable, the leave order for the treatment period.
  • Legal Heir Certificate: In case the claimant is a legal heir of the beneficiary.
  • Death Certificate: If the claim pertains to a deceased beneficiary.

How to Register for MMLSAY

  • Visit the Official Portal: Go to mmlsay.assam.gov.in.​
  • Beneficiary Registration: Click on the “Beneficiary Login/Registration” option.​
  • Enter Details: Provide your employment type, PAN number, and mobile number to validate your account.​
  • Fill Application Form: Complete the form with personal details and upload necessary documents such as Aadhaar card, PAN card, address proof, and passport-sized photograph.​
  • Submit and Acknowledge: After submission, an acknowledgment with a reference number will be generated. You will receive SMS and email notifications regarding the status of your application.
